Treasury Forms Information:

The form years available in this search are listed in the “Tax Year” drop-down.

Treasury provides forms in Adobe Acrobat PDF format for downloading and use. There is no computation, validation, or verification of the information you enter. Some information may need to be handwritten (signatures, for example). For security reasons, you should press the "Reset Form" button located on the top right corner of the form after printing your completed form to clear your personal data. Michigan Treasury Online (MTO)

Michigan Treasury Online (MTO) is available for Registration, Sales, Use and Withholding (SUW) tax years 2015 and beyond. MTO is free and provides secure 24/7 online access to registered Treasury business accounts. Some SUW forms are required to be submitted electronically through MTO or e-filed. These designated forms will be specifically identified in the search results column with a link to MTO.

Note: Your registration information and other forms submitted through MTO will be processed much faster than if paper filed.

Search Tips

Use this option to browse a list of forms by tax area. For example, if you are interested in Individual Income Tax forms, select that tax area and the tax year from the drop-down list. Click “search” to view results.

Use this option to browse a list of forms. This search first filters by the tax year selected. A second filter (tax area) is not mandatory, but highly recommended. When you open the form number drop-down, the forms displayed are based upon tax year and tax area if chosen. Select the appropriate form number from the drop-down list and click “search” to access that form.

Use this option to browse a list of forms by entering a key word or phrase that describes the form you need. For example, if you know the form has the word "change" in the title, enter "change" in the form name/key word box, select the tax year from the drop-down list and click “search” to access that form. The search results will include all forms that have the key word/phrase in the name of the form. For best results, do not enter any punctuation (no spaces or hyphens).

Use this option to filter/narrow down the existing results being displayed. By entering a form number or key word(s), the total number of results available will be filtered again. For example, if you search using tax year 2023 and Individual Income Tax as the "Tax Area", 40+ form results will be displayed. If you enter the word “Income” in the "Form Name/Key Words" box, only the forms that include that key word in the title will be displayed in the search results.
